Run the `Utilities/Scripts/clang-format.bash` script to update
all our C++ code to a new style defined by `.clang-format`.
Use `clang-format` version 3.8.

6a661f0603 fixed the
locale used for message output but at the same time broke the locale
used for filename encodings.
This commit preserves LC_CTYPE in the presence of LC_ALL.
